We are an NDIS registered provider of  Disability Supports, Supported Independent Living. Work Skill Development, Coordination of Supports and Specialist Support Coordination as well as Counselling services.

Our practice primarily focuses on building participant capacity through the development of knowledge to inform choice and control over NDIS services. Our engagement with you is individualised, goal orientated and collaborative to ensure that you achieve the very best outcomes from your NDIS Plan. 
 
Our team has lived experience within the disability sector and consists of accredited Social Workers (AASW) and Counsellors (ACA). Our staff have a vast array of experience in the Mental Health, Disability, Education, Housing and Not-for-Profit sectors.

We understand how complicated and overwhelming it can feel to navigate and access services under the NDIS and we know you and your goals are unique; so, our support is unique too. Collective Care Supports is based on the Sunshine Coast catering to Moreton Bay region, including Gympie.
