This support will assist you to build the skills you need to understand, implement and use your plan.

 

A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community

Specialist
Support 
Coordination

Level 3

A specialist support coordinator will help to manage any challenges in the support environment and ensure a consistent delivery of service. This is available for participants who require a higher level of support coordination and support.

Supported 
Independent
Living

Supported Independent Living (SIL) is help with or supervision of daily tasks to help you live as independently as possible, while building your skills. SIL is paid personal supports and commonly used in shared living arrangements. It’s for people with high support needs, who need help at home almost all the time.

The Disability and Community Support Worker works with people with a disability in achieving individual goals, dreams and aspirations and enhancing the individual’s participation and inclusion in the community.

Workplace Champions

Supports in the workforce

Workplace Champions Mentor provides employment support to people with a disability by identifying and working towards employment outcomes. Implementing person centered plans and actions to support participants work towards key milestones and develop skills to find and maintain meaningful employment.
